Once you receive your kit, attach a recent photograph of yourself to your paperwork. and fill out
your identification and contact information.
You may request kits for everyone in your household and consider putting them in multiple vehicles
or dwellings. (Hence it is important that you attach a photo to your paperwork; especially if you are
storing packets for more than one person in the same storage space.)

In Automobile:

STICKER (Decal) Placement:

If you put the yellow decal in an automobile, affix it to the glass on the lower
left corner of the rear side window on the left (driver’s) side of vehicle. The
window sticker will alert a first responder to open your glove compartment to
find your paperwork.

Medical Information Placement:
C
Maintain your medical information in the yellow folder/envelope in your glove
compartment. It will alert the first responders of any medical restrictions, or
existing conditions. They can relay your information to the hospital before you
arrive at the Emergency Room. If you are transported to a hospital, your yellow
folder containing your medical history will accompany you .

If you sell the automobile or move, remove yellow dot decal.

In Home:

Place a yellow sticker inside your main entrance at a location where it will be
clearly visible when opening your front door.

Store your yellow envelope/folder conspicuously in the freezer compartment
of your refrigerator.

This program will work best for those who keep their medical information updated. It
will require more effort for those who have pre-existing medical conditions or who are
currently taking medications (which should be identified in detail.)

maintaining it.

We at the Sheriff’s Office do not keep a copy of it for you. The plan was devised to provide a proxy for you
--to speak for you to the emergency responder(s) attempting to help you. The program works as long as it is
kept simple for everyone involved.
