Q: When
I enter the hospital, a discharge
planner comes to see me and often my
family members to talk about care after
leaving the hospital; and this happens
before I am even well enough to think
about leaving the hospital. Why is this
necessary?
A:
The hospital discharge planner helps to
insure that everyone who is in the
hospital and leaves the facility goes
into a safe situation based on their
needs for care upon being discharged.
Many people return home with little or
no help other than family assisting for
a few days or weeks, but a growing
number need significant care when they
return to their home; or they may need a
rehabilitation program or the services
of a residential care facility for a
period of time. The discharge planner
assists the patient and family to plan
for how and where the care needed will
be provided for the patient. It is
important to have these discussions
shortly after admission so a plan is in
place by the time the patient is ready
for discharge.
