Staff is available to provide information and counseling on eligibility for government benefits, such as: Medicare Savings Programs, Medicaid, Food Stamps, HEAP, EPIC, and various tax reductions. In addition there is a staff person available to discuss long term health insurance options.

A community-based, non-medical program offering ongoing case management and home care services to older adults, incorporating a cost-sharing feature based on a client’s income and housing expenses. A comprehensive, independent assessment by a case manager determines client’s need and ability to pay. Any Delaware County resident, 60 or over, having difficulty with activities of daily life (dressing, bathing, meal preparation, eating, shopping, laundry, or housework) is eligible.

This program provides assistance with home care management for Medicaid recipients that require such assistance to remain at home, with the approval of their physician. Case management is the process that links the client with the needed service such as care aides and nursing services. Staff also provide the consumer with available options and provide information on the costs of various services, adult homes and/or nursing homes. They can assist with the coordination of nursing home placements from the community. This program can do Emergency Response assessments and referrals, coordinate transportation to medical appointments through Delaware Opportunities or ambulance services, and assist with recertification for Medicaid and Food Stamps.
