Certifies and oversees Delaware County family type adult homes for up to four residents. The service also provides information and referral. This service is for prospective residents or operators, and is free.

County agency nurses make a home visit to Delaware County residents in need of home care or having difficulty managing in their current setting. Evaluate and give information on the assessed level of care and discuss options available in the community. Completion of paperwork for nursing home placement as appropriate and evaluation of nursing home residents who wish to return to a home setting.

Assists low to moderate income families in locating affordable housing in the Deposit area. Manages apartment facility for limited income clients.

Adult/Family Type Homes

Adult Homes provide five or more residents with rooms, meals, help with daily living and some degree of protective supervision. A resident may need assistance with the management of medications, but must be alert enough to take his/her own medications. These homes are licensed and monitored by the NYS Health Department, Bureau of Licensure and Certification. Some may offer short-term respite beds.

Hampshire House is a licensed adult care facility with 65 beds in their facility. The rooms are private and semi-private, cable ready, with individual phone capacity. There are 24-hour services which include Registered and Licensed Nurses, Certified Home Health and Personal Care Aides, Physical Therapy, and Occupational Therapy. There is medication supervision, daily housekeeping and weekly laundry service. There is a heated indoor pool, several lounges, and a computer room. There is a dining area where residents can have a restaurant style meal. Hampshire House also has an activities director for classes, outings and events. Call for more information.

Up to 21 residents. Private Pay, SSI, and third-party payment.  Other options: The NY Office of Children and Family Services certifies boarding homes and rooms in family homes with certain services for elderly residents. The Delaware County Department of Social Services inspects and monitors these homes. For a list of homes in the area with four beds or less, contact DSS or Office for the Aging.  If you are considering an adult home for yourself or a family member, an assessment by the Office of Long Term Care is recommended as a first step. Assisted Living Facilities

Assisted Living Programs (ALPs)
are licensed by the NYS Dept. of Health. They provide the same services as an Adult Care Facility along with services of a licensed home health care agency. At this time, only ALPs may accept Medicaid.

Robynwood is comprised of a 30 bed ALP (Assisted Living Program) and 17 AH (Adult Home) beds. The rooms are semi-private with full bath, cable ready with individual phone capacity. Services include Registered and Licensed Nurses, Certified Health and Personal Care Aides, Physical Therapy, Speech Therapy, Occupational Therapy, a fitness program and you have the option of choosing your own physician. Rates: SSI, Private Pay & Medicaid reimbursable. There is 24 hour accessibility, the client calls in and their call is forwarded directly to the person on call.

Nursing Homes

Skilled Nursing Facilities
provide 24-hour medical care. A variety of professional and licensed staff assure that each person’s needs are met. Care is planned using input from the resident, family, physician, and facility staff. Care may be provided long-term or short-term depending on the resident’s needs. Some have amenities such as exercise rooms, gardens, and pets. These facilities may be paid for privately, by long-term care insurance, by Medicaid, see Chronic Care Medicaid and in special short-term circumstances by Medicare. Some facilities have Special Needs Units that are tailored to the needs of the residents with specific needs. Licensed by NYS Dept. of Health.

Dementia units
are tailored to residents with memory related problems including Alzheimer’s disease. These units offer specialized programming, staffing patterns and safety features.

Rehabilitation units
are tailored to residents with short term rehabilitation needs. The goal is to help a person regain his level of functioning prior to an illness, injury, or surgery. The type of care and length of stay are tailored to the person’s individual needs. Medicare may pay for this type of care up to 100 days.

Respite Care:
Provides temporary supportive care in a nursing facility or adult care facility for those who normally live in the community, but whose caregivers need relief (also known as short-term scheduled care).

Countryside has 160 beds in their facility which includes an Enhanced Care Dementia unit and a Short Term Rehabilitation Wing. There is 24-hour services which include Registered and Licensed Nurses, Certified Nursing Assistants, Physical Therapy, Speech Therapy, Occupational Therapy, a fitness program and you have the option of choosing your own physician. For short-term rehab and certain skilled nursing care, Medicare may pay for this service.

Mountainside has 82 beds in their facility. The rooms are private and semi-private with full bath, cable ready with individual phone capacity. There are lounges and sitting rooms and a 24-hr café for visitors and residents. Mountainside has on staff a podiatrist, dentist, and optometrist. There are 24-hour services which include Registered and Licensed Nurses, Certified Health and Personal Care Aides, Physical Therapy, Speech Therapy, Occupational Therapy, and there is a specialization in geriatric care along with specialized services such as podiatry. There is also a fitness program. A PRI and Screen are needed for admission along with admission application. Rates: Private Pay & Medicaid reimbursable. For short-term rehab Medicare may pay for this service.

New York State Veterans’ Home

New York State Veterans’ Home is a 242-bed facility that provides long term care and short term rehabilitative care. Supervised medical care and nursing services are provided on a 24-hour basis. Support programs include social services, recreational activities, physical and occupational therapy and other rehabilitative services. Special diets are provided through a registered dietician on staff. A PRI and Screen are needed for admission along with application. The applicant must have entered active duty from the State of NY or be a NY State resident for one year prior to admission. The Veteran must also have had an honorable discharge from the US Armed Forces and have at least 30 days of active service. Rates: Medicaid and Medicare for short-term rehab.

Otsego Manor has 174 beds with two additional respite beds in their facility. 136 beds are in private rooms and 20 beds are in semi-private rooms. Rehab rooms are cable ready and all rooms have individual phone capacity. There are 24-hour services, which include Registered and Licensed Nurses, Certified Health and Personal Care Aides, Physical Therapy, Occupational Therapy, and Palliative Care. A PRI and Screen are needed for admission along with an admission application. Rate: Private Pay & Medicaid reimbursable and will accept Veteran's benefits for eligible veterans. For short-term rehab, Medicare may pay for this service.

There are 122 beds with a high staff-to resident ratio with dedicated doctors, nurses, therapists and support staff. Meals and menus are supervised by a registered dietician. There are a wide roster of activities, outings and special programs designed to engage residents. As a Medicare and Medicaid certified site, it’s also a skilled nursing facility with 24-hour services which include Registered and Licensed Nurses, Certified Health and Personal Care Aides, Physical Therapy, Speech Therapy, Occupational Therapy, and a fitness program. Short-term respite is paid by Medicaid or Private Pay and medical day care is paid by Medicaid or Private Pay.

St. James’ Manor, a senior living community for residents 62 years of age and older. The mission of St. James’ Manor is to provide quality of life for its residents through the design and stewardship of moderately priced, independent living facilities, with support services and programs. Situated on almost 10 acres, the complex consists of 38 one and two-bedroom apartments and 8 townhouse-style duplex units with attached garages. Garages are also available for those who live in the apartment complex. All apartments and duplex units are on one level and have full kitchens with appliances provided and maintained by the Manor. One meal a day is provided for residents along with weekly laundering services, apt. cleaning every other week, daily well-checks, emergency response system and much more.
