On-call nurses are available evenings, weekends and Holidays by calling 607-746-2336.

This program is for frail, disabled and chronically ill persons of any age who are medically eligible for admission to a nursing facility, but can safely remain at home. They must have a doctor’s order for service. The LTHHCP offers a coordinated plan of medical, nursing and rehabilitative care provided at home. In addition, the LTHHCP can arrange or provide for home-delivered meals, emergency call systems, adult day care, moving assistance, and home improvements. Service may be reimbursed by Medicare, Medicaid, private payment, and some health insurers. Must be eligible for chronic care Medicaid when billing Medicaid for services.

Chronic Care Medicaid

This program will pay for skilled nursing care for eligible recipients in appropriately certified facilities or who qualify for the Long Term Home Health Care Program (LTHHCP). For individuals or married couples looking for "nursing home without walls" care, the LTHHCP program allows them to receive care at home paid by Medicaid.

Spousal Impoverishment Budgeting

Analyzes the treatment of income and resources of a couple when determining eligibility for an institutionalized spouse. Spousal impoverishment budgeting rules protect some of the couples income and resources the support of the community spouse. These rules allow the community spouse of a waiver to retain more income and resources than he or she might under Medicaid community budgeting rules or nursing home resident.

This funding is designed to be the final source of assistance.  All other forms of assistance must be exhausted or denied.  The application is processed through Office for the Aging.

Provides in-home assessment to determine possible accessibility adaptations, recommendations and referrals for home modifications.

Home Rehabilitation and health and safety repairs. Emergency repairs for elderly homeowners where health and safety are concerns. Various housing rehabilitation programs provide grants to bring property to code and address health and safety issues.

This program helps low income and elderly residents of Delaware County to weatherize their homes to save energy. This help includes various types of insulation, weather stripping, sealing leaks, preventing drafts, caulking, replacing broken glass, puttying windows, repairing and installing new storm windows and exterior doors, skirting trailers and sealing foundations. Renters and owners qualify if they meet income guidelines.

This program links clients with Delaware Opportunities weatherization program, assisting them with energy and limited home repair needs. This can include insulation, weatherstripping, roofing repairs, furnace cleaning and repair, leaks in foundation, possible replacement of storm and prime windows and doors, replace broken glass and caulking.
